The family and friends already in an older adult’s life are their greatest asset. The Guardian coordinates this care circle, supports the implementation of the care plan, and helps monitor progress.

Your Care Circle keeps a close, steady eye on how your action plan is working. Through simple check‑ins, they track your wellbeing, notice early changes, and make sure services are delivered with quality and care. Together with the Guardian, we adjust the plan when needed so your support always stays relevant and responsive to your life.
